I am sorry but we do not have scantron forms in the Library.  You might try the bookstore on Monday.  The phone number is (516) 572-7168. You should call the Chemistry department office Monday morning and ask the secretary where to locate a form.  That number is  516.572.7580.

I would also suggest you call the Dean of Students Monday morning to see if anyone there has a solution. The phone number is
